There are several ways to submit the form to us:
- Print the form, complete the information, then scan and fax or email it to us.
- Bring the form with you, or mail it to our office.
- Open the form in a .PDF reader, then complete the information and click the “Email to CKD’s” button at the bottom.*
* Please note that you cannot email the form directly from our website; you must open the form in a .PDF program or reader application. This may require saving the form to a folder, and then opening it from there. Or look for an “Open in Acrobat” button the side. Or else try right-clicking on the form link below and see if there is a choice to use your .PDF viewer, such as Adobe Acrobat.
Once the application has been completed, click the email button at the bottom. Depending on your .PDF program, an email may open with the form already attached. Or a content box may open from which you can choose your email program.
If you use webmail, like Gmail or Yahoo, you may need to set up your email account within the .PDF reader if you have not done so before. If so, select the webmail account type (Gmail, etc.) and enter your own email address and password. If you are using Gmail, the “sent” email may open in your drafts folder; if so, open the email and then send it.
Completing the Form
The form is “fillable” online. In the top section, please check one of the boxes at the top, then specify whether or not your application is for joint credit, the loan amount requested, and the number of monthly payments desired. Complete the Applicant Information (2nd section) of the form. Then sign and date at the bottom. Thank you!
